Before I show you how to create a realistic shadow, you need to know a few things about shadows in general:
Shadows appear on the other side of objects, away from the light source. Shadows don’t always appear on the edges of objects.

To control the appearance of shadows, increase the contrast of the image.

Shadows can result in a realistic looking shadow that blends with the rest of the picture. To create a realistic shadow, you need to understand a few basic tools.
1. Display your image in the Editor window.
2. Create a new document in Photoshop.
3. Choose Filter ▸ Blur ▸ Gaussian Blur.
4. Set Radius to 5 and Press Enter.
If the blur is too soft for the effect you want, set the Radius value larger.
5. Apply Gaussian Blur to the image.
6. Duplicate the original layer.
7. Use the Brush tool to create a soft shadow on the layer.
8. Change the Opacity of the shadow layer to 15 percent in the Layers panel.
You may need to press Ctrl+L (Windows) or +L (Mac OS) to load a new document in case you switch back to the original layer in Step 3.

To determine the hemodynamic significance of color Doppler findings in left ventricular diastolic dysfunction, 80 consecutive patients (43 male, 37 female; average age 56 +/- 10 years) were examined. Of the 80 patients, 45 (56%) were classified as having normal diastolic function by color Doppler echocardiography. Left ventricular wall motion abnormalities were found in 32 patients (40%). Of these, 28 (66%) were regarded as having diastolic dysfunction. Of the remaining 32 patients, 26 (81%) were classified as having normal diastolic function, but 6 (19%) were classified as having diastolic dysfunction. Of the 6 patients classified as having normal diastolic function, 6 had a ring signal of 5, 3 had malalignment of 5, and the remaining 3 had peak systolic flow of 3. Of the 26 patients classified as having diastolic dysfunction, 1 had peak systolic flow of 3, and the other 25 had peak systolic flow of 0 or 2. Left ventricular outflow tract flow was identified in 0 to 2 patients. Of these, only 1 patient had a left ventricular outflow tract systolic flow of 2.
